Home and Community Care
This agency is funded by Commonwealth and State Government initiatives.
Home And Community Care (HACC) funding is designed to provide basic services to help people avoid inappropriate admission to long-term residential care. The government also provides Aged Care Packages through Silver Chain, and subsidises hostel or nursing homes for people with higher needs. Sometimes these services become more appropriate for a client's needs than those that Shire of Manjimup HACC can provide. The choice remains with the client to stay at home, but our services are limited to the following:
- Heavier house cleaning, including washing and shopping (1 - 2 hours per week or fortnight, depending on the circumstances).
- Wood chopping - ½ hour per week maximum, except in special circumstances.
- Home maintenance services, including lawn mowing and gardening (up to 2 hours monthly) - for safety purposes.
- Local transports for medical reasons as needed and/or shopping once a week (Thursdays), and
- Transport to the nearest medical specialists where those services are not available locally, and you have a referral from your local medical practitioner.
Our CRC funding is to support Carers who are looking after a frail aged or disabled person - under this program we organise respite services in the home and outings to provide carers with support and relief. We also target isolated people and ethnic minority groups.
We are registered service providers for the Veterans' Home Care Program. The process: the veteran rings Veterans' Home Care Assessment authority to request services, that agency authorises us to provide a service and we bill the Department for authorised services delivered. Services provided under this program include:
- Domestic assistance - as need is assessed - usually a maximum 1.5 hours/fortnight
- Home Maintenance - maximum 15 hours per annum
- Respite - as needed
