Eligibility for service is subject to Manjimup Home and Community Care (MHCC) guidelines. You do not qualify for MHCC support on the basis of your age, but rather on the level of your frailty or disability and on your ability to carry out essential everyday activities.
Manjimup Home and Community Care services require a free assessment by the Regional Assessment Service (RAS). The assessment is simple and informal and involves a chat between yourself, maybe your carer and the assessor, to decide what your needs might be. You can telephone the Regional Assessment Service on 1300 785 415 to make your RAS assessment appointment. If you already have your RAS assessment or require some assistance to do so please call Manjimup HCC directly on 9771 6900.
If you have higher care needs you will be referred to an Aged Care Assessment Team (ACAT). An ACAT can approve you as eligible for MHCC or a Home Care Package Level 1 – 4. Once you have an ACAT assessment, you can contact Manjimup HCC directly on 9771 6900 to begin to arrange your care.
