Aged Care - Support at home

Support at home (SAH) are personalised support services designed to help seniors live happier and healthier lives at home. Click here to see the support at home price list.

If you’re over 65, or an Aboriginal or Torres Strait Islander over 50, you could be eligible. Funded by the Australian Government, Support at Home available in eight levels. If you’re approved for Support at Home, you will contribute a percentage (determined by Services Australia) towards the cost of your care.

The government will pay these funds into your package for you to use as you like.

The beauty of Support at Home is that it’s all up to you!

Based on your allocated package, you can choose services that suit your needs and budget. When you join Manjimup Home & Community Care, you’ll be matched with a dedicated Care Partner who understands your goals and helps you make the most of your funding - supporting your home, health, and wellbeing.

Day Clubs and Activities
We run a range of clubs and activities at the Wellness & Respite Community Centre offering your loved one:

  • Quiz & crafting days
  • Guest speakers
  • New skills to learn
  • International lunches
  • Dementia specific club
  • Community outings

Dementia Care
Family carers can enjoy a short break, confident in the knowledge that their loved one is enjoying a wonderful time with caring, staff and friends, enjoying:

  • Activities
  • Time with friends
  • Wellbeing time

Domestic Assistance
We're here to help you around the house. Whatever the task, we have a skilled team who are up to the job:

  • House cleaning
  • Mopping
  • Bed making
  • Vacuuming
  • Sweeping
  • Laundry

Home and Garden Maintenance
Our outdoors team will assist you or your loved one in maintaining the house and garden. We offer:

  • Weeding
  • Mowing
  • Light pruning
  • Planting

Meals on Wheels
Our meals on wheels service make it possible for you or your loved one to stay home and relax. Let us come to you, offering:

  • 2 - 3 courses (entree, main, dessert)
  • Friendly volunteers
  • Hot, delicious & nutritious meals
  • Delivered to your door

Palliative Care
We work with you, your loved ones and your palliative care team to plan your end of life journey and provide assistance, support and holistic care. We offer:

  • Family and loved one support
  • Coping assistance
  • End of life care at home

Personal Care
Personal care is a sensitive topic, which is why we offer bespoke services that are specific to you or your loved one's needs. These services include:

  • Oral health
  • Hair care
  • Shaving
  • Nursing
  • Make-up & beauty
  • Showering & dressing
  • Continence management

Social Support
Our team are always on hand to help you or your loved one stay socially connected in the way that is important. We do this by engaging in the things you or your loved one enjoy:

  • Reading
  • Board games
  • Conversations
  • Shopping
  • Playing music
  • Sports outings

Transport
Provision of transport to appointments, visit friends, shopping and even specialist trips to Bunbury. Please enquire regarding driver availability, prior notice required.
